Before starting the application you will need to ensure that:

  • All brochures and forms are read carefully and retained.

 To join, you will need:

  • Eligibility, please be aware that as an education union-based fund, UniHealth Insurance has membership eligibility criteria for joining.
  • Details of those covered (birth dates).
  • Medicare Card if you plan to claim the Australian Government Rebate as a reduced premium.
  • If you are transferring from another fund you can authorise UniHealth Insurance to obtain a clearance certificate on your behalf.  Alternatively, download and complete a Clearance Certificate Request form.
  • Account details will be required for direct debit contributions and benefits payments.
